To become a Next Wave appointed producer, please mail, fax (619-234-6500) or scan and email ( submissions@nextwaveins.com ) the completed and signed Producer Agreement, along with the W-9 Form and a copy of your Broker/Agent License(s) and a copy of your firms E&O Declarations page.
